Genital herpes is a sexually transmitted infection (STI) caused by the herpes simplex virus type 2 (HSV-2). Herpes simplex virus type 1 (HSV-1), traditionally associated with cold sores, increasingly causes genital infections as well.
The herpes virus spreads through direct skin contact with infected the areas during vaginal, anal, or oral sex. Genital herpes can be transmitted even when there are no visible symptoms.


Patients suffering from genital herpes can remain asymptomatic or experience very mild symptoms which are unnoticeable. When symptoms do appear, they typically include painful fluid-filled blisters on or around the genitals, rectum, or mouth that break open, leaving tender ulcers that may take 2-4 weeks to heal. The first or initial outbreak is often the most severe and can be accompanied by symptoms including fever, body aches, and headache.
After this initial infection, the virus remains dormant in the nerve cells, and causes recurrent outbreaks when triggered. These subsequent attacks are usually shorter and not as severe than the initial outbreak. The trigger factors for these recurrent outbreaks include stress, illness, fatigue, hormonal changes, and weakened immune.
Dr Weightman can provide antiviral medications which can effectively manage genital herpes. These include Aciclovir (Zovirax), Valaciclovir (Valtrex) and Famciclovir (Famvir). Aciclovir was the first antiviral developed for herpes. It works by interfering with viral DNA synthesis, preventing the virus from replicating. Valaciclovir is a prodrug that converts to aciclovir in the body but offers better absorption and less frequent dosing. Famciclovir (Famvir) also works in the same way as aciclovir and valaciclovir.
These medications can be prescribed to treat the outbreaks when they occur, reduce the outbreak frequency and viral shedding and reduce transmission risk to partners.
Although not specifically indicated for genital herpes treatment, Instillagel is a sterile lubricating gel containing lidocaine (local anaesthetic) and chlorhexidine (antiseptic). It can sometimes provide temporary pain relief associated with herpes lesions, but it does not treat the viral infection itself.
Aciclovir is used to treat viral infections such as cold sores and genital herpes. It works by stopping the virus from multiplying, helping symptoms clear more quickly. It’s usually taken several times a day for a short course, although some people may use it longer-term to prevent recurring outbreaks.
Common side effects of aciclovir tablets include headache, nausea, vomiting, and diarrhoea. Some patients report dizziness or fatigue, particularly at higher doses. Skin sensitivity, including a rash or photosensitivity (increased sensitivity to sunlight), can occur. Neurological effects such as confusion or agitation are rare and more likely at high doses or in individuals with impaired kidney function. It is important to maintain adequate fluid intake whilst taking aciclovir to reduce the risk of renal complications.
Valaciclovir tablets
Valaciclovir is an antiviral medicine used to treat infections caused by the herpes virus, including cold sores and genital herpes. It works by stopping the virus from multiplying, which helps reduce the severity and duration of outbreaks. It’s usually taken as oral tablets once or twice daily for a short course, depending on whether it’s being used to treat an active outbreak or to prevent recurrences. For best results, treatment should be started as soon as symptoms appear.
Valaciclovir, which is converted to aciclovir in the body, has a very similar side effect profile. Headache and nausea are the most commonly reported effects, along with vomiting, diarrhoea, abdominal pain, and dizziness. Skin reactions and photosensitivity have also been noted. Neurological side effects, although uncommon, are more likely at higher doses or in those with reduced kidney function. As with aciclovir, good hydration is recommended throughout the course of treatment.
