Altitude sickness or AMS for short, is a condition that affects climbers who climb to high altitudes. As the climber’s altitude increases, the atmospheric oxygen pressure and the oxygen saturation in the blood reduces potentially leading to symptoms.
Normally, the body requires time to adapt to these changes, but when the climb is too rapid, symptoms may develop.
There are three different forms of altitude sickness. Mild to moderate AMS with symptoms being headache, fatigue, dizziness, nausea, vomiting, loss of appetite, and sleep disturbances. High-altitude pulmonary edema (HAPE) is symptomised by extreme fatigue, breathlessness at rest, cough (potentially with pink frothy sputum), chest tightness and cyanosis. High-altitude cerebral edema (HACE) causes severe headache, confusion, ataxia (loss of coordination), hallucinations, and potential coma.
HAPE and HACE are severe, potentially life-threatening conditions and would require immediate medical intervention.
Altitude sickness is managed by prevention, ideally by having a gradual ascent and allowing time for acclimatisation, but also by maintaining hydration and avoiding alcohol and sedatives.
However, Dr Weightmans can prescribe acetazolamide as prophylaxis for high-risk individuals or those with previous episodes.
Mild to moderate AMS is treated by descending to a lower altitude, resting, hydration, and medicines to manage symptoms. Painkillers and anti-sickness medication may help with the headaches and nausea. Patients who suffer from pulmonary hypertension can be given nifedipine to reduce this. Cerebral inflammation is reduced in HACE by administering Dexamethasone.
Certain populations face higher risks, including those with pre-existing cardiovascular or pulmonary conditions, previous episodes of altitude sickness, rapid ascent histories, and higher exertion levels at altitude. These individuals should take extra precautions and consider medical consultation before traveling to high-altitude destinations.
