What is Foundayo and who makes it

Foundayo is the brand name for orforglipron, a once-daily oral GLP-1 receptor agonist developed by Eli Lilly - the same pharmaceutical company behind Mounjaro (tirzepatide). Originally discovered by Chugai Pharmaceutical and licensed to Lilly in 2018, orforglipron has since been through an extensive Phase 3 clinical programme across multiple countries.

Foundayo works by mimicking glucagon-like peptide-1 (GLP-1), a naturally occurring gut hormone released after eating. By activating GLP-1 receptors, it reduces appetite, improves feelings of fullness, and slows stomach emptying - the same fundamental mechanism shared by Wegovy and Mounjaro. Consequently, the Foundayo pill sits in the same drug class as the best-known weight loss treatments currently available.

Notably, the MHRA granted Foundayo a dual approval on 10 August 2026 - both for weight management in adults with obesity and for the treatment of type 2 diabetes. This dual indication sets it apart from the Wegovy pill, which currently holds approval for weight management only.

Why the Foundayo pill is different from the Wegovy pill

Both Foundayo and the Wegovy pill are once-daily oral GLP-1 weight loss tablets. However, the practical experience of taking them is significantly different - and for many patients, Foundayo's approach will suit daily life considerably better.

The Wegovy pill produces higher average weight loss in trials. Nevertheless, real-world outcomes depend heavily on how consistently a patient takes their medication. Strict morning fasting routines are not always practical - particularly for people with early starts, irregular schedules, or those who simply cannot commit to the same morning window every day. In those cases, Foundayo's flexibility could translate into better real-world adherence and, therefore, better outcomes over time.

The science behind Foundayo - why no food rules are needed

Understanding why the Foundayo pill needs no food restrictions requires a brief look at what makes it structurally different from every other oral GLP-1 available.

The Wegovy pill is a peptide drug - meaning semaglutide, its active ingredient, is a protein-based molecule. Peptides break down in stomach acid and digestive enzymes before they can be absorbed. To overcome this, the Wegovy pill uses SNAC technology - a chemical agent that protects semaglutide from the stomach environment and helps it absorb through the stomach lining. That process requires a completely empty stomach and specific water conditions to work correctly.

Foundayo is fundamentally different. Orforglipron is a small-molecule, non-peptide drug. Small molecules survive stomach acid naturally, without any protective technology. As a result, orforglipron absorbs effectively regardless of whether food or other liquids are present in the stomach. There is no absorption mechanism to protect, no timing window to maintain, and no fasting requirement to follow.

Clinical evidence - what the ATTAIN-1 trial showed

The primary evidence base for Foundayo in the UK comes from the ATTAIN-1 Phase 3 clinical trial, a 72-week, randomised, double-blind, placebo-controlled study involving 3,127 adults with obesity or overweight without type 2 diabetes. Results were published in the New England Journal of Medicine in September 2025 and formed the basis for both the FDA approval in April 2026 and the MHRA approval in August 2026.

In terms of raw trial efficacy, the ranking runs Mounjaro first, Wegovy pill second, Wegovy injection third, and Foundayo fourth. However, this ordering tells only part of the clinical story.

For patients who need to avoid injections, the choice narrows to the two oral pills. Between them, the Wegovy pill produces higher average weight loss but demands a strict daily morning routine. Foundayo produces somewhat lower weight loss in trials but imposes no routine at all. Moreover, Foundayo carries the additional approval for type 2 diabetes - making it the more versatile option for patients managing both conditions simultaneously.

Ultimately, the right choice depends on the individual patient's clinical profile, lifestyle, and what they can sustain over the long term. All four options are available at Dr Weightmans - or will be, in Foundayo's case, as soon as stock arrives.

Foundayo dosing schedule

Like all GLP-1 treatments, Foundayo uses a gradual dose escalation schedule. Starting at a low dose and increasing slowly - at a minimum of one month per step - allows the body to adjust and reduces the likelihood and severity of gastrointestinal side effects.

The titration schedule means reaching the maintenance dose takes approximately five to six months. This gradual approach is clinically intentional. Prescribers at Dr Weightmans will manage the dose escalation individually - keeping patients at a particular step for longer if side effects require it, or reviewing progression at each re-order.

Who is eligible for Foundayo in the UK

The MHRA licensed Foundayo for two patient groups. First, adults with a BMI of 30 or above. Second, adults with a BMI between 27 and 30 who have at least one weight-related health condition - such as hypertension, dyslipidaemia, type 2 diabetes, obstructive sleep apnoea, or cardiovascular disease.

These criteria broadly match those of the Wegovy pill and Mounjaro, meaning patients who qualify for one of those treatments will generally qualify for Foundayo too. As with all GLP-1 treatments, a full clinical assessment by a UK-registered prescriber is required before any prescription is issued.

Foundayo is not suitable for everyone. Specifically, the following patients would not be appropriate candidates:

Side effects

Additionally, the MHRA has noted it will keep the safety and effectiveness of Foundayo under close review as use begins in the UK - standard practice for newly approved medicines. When will Foundayo be available in the UK to order

Foundayo UK received MHRA approval on 10 August 2026 but is not yet available in pharmacies or from online providers. A newly approved medicine typically takes several weeks to move from regulatory approval to commercial availability - as stock reaches wholesalers, dispensing systems are set up, and providers complete their clinical governance processes.

A useful reference point is the Wegovy pill, which followed a similar path. The MHRA approved the Wegovy pill on 11 June 2026, and it became available through private providers approximately three and a half weeks later, on 6 July 2026. Based on this precedent, Foundayo could reasonably be expected to become available through regulated private providers in the UK by early September 2026, though no confirmed date has been announced.

Will Foundayo be available on the NHS

Not yet. MHRA approval licenses a medicine for use in the UK but does not automatically make it available through the NHS. A separate NICE health technology appraisal determines whether the NHS will fund and reimburse a treatment - taking into account clinical and cost effectiveness.

According to NICE, its decision on Foundayo for weight management is due on 18 November 2026. Until that decision is published, Foundayo will only be available privately. Furthermore, even if NICE recommends NHS funding, rollout typically follows a phased path and may initially be restricted to specific patient groups through specialist services.
